Geriatric Care Management

The Neighborhood House’s Geriatric Care Management Program provides comprehensive care management services to frail and homebound older adults each year in Community Districts 8 and 11, covering the East Side of Manhattan from 59th Street to 143rd Street including Roosevelt Island. Geriatric Care Clients must be sixty or older, frail and homebound.

The Neighborhood House is able to make a profound difference in the lives of these vulnerable individuals, helping them overcome crises and providing the services, support and community they need to remain independent, to feel connected, to achieve better outcomes and to live fuller and healthier lives. 

Geriatric Care Managers help clients access a very wide range of government benefits, private benefits and services including: Senior Citizen Rent Increase Exemption (SCRIE); Elderly Pharmaceutical Insurance Program (EPIC); Supplemental Nutrition Assistance Program (SNAP);  Access-a-Ride; Medicaid Managed Long Term Care (MLTC); Medicare supplemental programs; medical and psychiatric referrals; medical escorts; shopping escorts; volunteer programs; long-term supportive counseling; and stipendiary funding.

Care Managers also provide a wide range of additional services including: assessments for emergency assistance; housekeeping, exterminators and other interventions; home modifications; caregiver support; engagement and work with family to assist with long-distance caregiving; Alzheimer’s support; financial management; and referrals and help with accessing government benefits.
