Case Management


Lenox Hill Neighborhood House provides case management to seniors in Manhattan Community Districts 6, 8 and 11 on the East Side of Manhattan and on Roosevelt Island, 14th Street – 143rd Street, 5th Avenue to the East River. For our services in Community Districts 8 and 11, we work closely with our partners, The Carter Burden Center for Aging and Search and Care.

Our social workers and case managers help clients access myriad services that enable older adults to remain in the community and age at home.

National Council on Aging's Economic Security Initiative

 


We are a pilot community for the National Council on Aging's Economic Security Initiative, one of just eight organizations nationwide chosen by NCOA for this incredible two-year pilot project. The goal of the Initiative, funded significantly by the Harry and Jeanette Weinberg Foundation and Atlantic Philanthropies, is to increase economic security for low-income adults, age 55 and older, with incomes below 250% of the federal poverty line and to provide economic case management services to help thousands of low-income seniors nationwide achieve greater economic security.
