a range of coordinated programs to ensure a continuum of quality care

Lenox Hill Neighborhood House Senior Centers
Fun, friendship, and learning for adults age 60 and over. The centers
offer an exciting variety of educational, cultural and recreational activities
as well as hot lunch.

& Simple Estate Planning
Monthly Walk-In Clinic
The Legal Advocacy and Organizing Department of Lenox Hill Neighborhood
House provides an Advance Directives & Simple Estate Planning monthly
walk-in clinic at the Lenox Hill Neighborhood House Senior Center at Saint
Peter’s Church. The clinic offers guidance on Health Care Proxies,
Living Wills, Powers of Attorney, and Simple wills. The walk-in clinic
is held on the first Monday of every month.
Case Management Services
The East Side Case Management Consortium provides comprehensive case management services to older adults on the Upper East Side and in East Harlem. Lenox Hill Neighborhood House is the lead agency of this consortium, which also includes The Carter Burden Center for Aging, Stanley Isaacs Neighborhood Center and Search and Care. Homecare Services
An affiliate of the Neighborhood House, The Caring Neighbor (TCN) provides
Medicaid funded and EISEP subsidized home care, including nursing and
housekeeping services, to homebound elderly and disabled individuals.
Licensed by the New York State Department of Health, TCN serves over 550
clients from various sections of Manhattan, including Chinatown, Battery
Park, Roosevelt Island and the Upper and Lower East Side. For more information,

Day Services
Lenox Hill Neighborhood House’s social adult day programs enhance
the well-being of physically and/or mentally frail older adults:
Health Enhancement
Partnership (HEP) is a full-time day care program that brings otherwise
homebound older adults together for recreational and social activities.
Designed to serve the growing population of adults age 60 and over who
are physically or mentally frail, the HEP program provides exercise, music,
arts and crafts, cultural trips and other activities. The program is conducted
at the Lenox Hill Neighborhood House Senior Center located at 343 East
70th Street.

The CARE
Program (Center for Alzheimer’s Respite care for Elderly)
A specialized adult day program for persons with memory loss
from Alzheimer’s disease or related disorders. CARE is a unique,
innovative, small group activity program designed to provide a warm and
welcoming environment and meaningful activities for participants while
offering respite and support for their caregivers. The program is conducted at our 70th Street Senior Center at 343 E. 70th Street. For more

Support Services
for Caregivers (Project STAR - Support, Training, Advocacy and Respite)
Project STAR has announced a support group for adult children who are
caring for a parent meets weekly from 6:30 to 8:00 p.m. on Tuesdays. If
you are a member of Lenox Hill Neighborhood House or a member of our community
and you are having difficulty balancing the challenges of caring for a
parent over the age of 60, this group will supply a safe and supportive
environment in which to express concerns and seek help.
A support group for spouses meets weekly from 11:00 a.m. to 12:00 p.m.
on Thursdays.
A long distance caregiver group meets Monday evenings from 6:30 to 8:00
p.m. This group is for individuals who are caring for a parent or other
relative over the age of 60 who lives out of town.
There is some space available in all groups. Caregivers are welcome to
call about any of the groups if they are interested in joining. Support
groups and individual counseling services are available free of charge
for spouses, children, relatives and other unpaid caregivers of frail
adults over age 60. We also offer community education forums, bereavement
counseling, weekly swim & gym nights, and respite homecare services.

Project STAR announces
the formation of a Bereavement Group for spouses and partners. Coping
with the loss of your spouse or partner can be a difficult process that
does not have to be done alone. This group provides a safe space to discuss
these feelings and concerns with others. The group will meet on Thursdays
from 6:30 – 8:00 PM for ten weeks beginning March 10, 2005. Click

Transportation
For seniors who are unable to use public transportation, Lenox Hill Neighborhood
House's wheelchair-accessible buses provide rides to senior centers, medical
appointments, grocery shopping, trips and recreational activities. Transportation

Weekend Connections
Together with the Burden Center for the Aging, Search and Care, and Stanley
Isaacs Neighborhood Center, Lenox Hill Neighborhood House runs a weekend
activity and transportation program for senior citizens on Manhattan's
East Side. Transportation is provided for a wide variety of excursions
to museums, performances, churches, synagogues and other destinations.
